The Herzog Center for Connected Care was established with a vision to address and mitigate healthcare disparities, especially in rural areas of Missouri and Southern Illinois, as well as underserved communities in and around the Greater St. Louis area.
Our first initiative is set for providing care in support of maternal health to help ensure healthier pregnancies in our region.

Why I Created the Herzog Center for Connected Care:
Hello. My name is Jacob Herzog. Whitey Herzog was my grandfather and a source of endless inspiration for me and our entire family. Throughout my career as a nurse, telehealth administrator and healthcare advocate, I have seen the challenges faced by underserved communities in accessing quality healthcare. By creating this center, I aim to bridge the digital divide and provide comprehensive, innovative care solutions that empower and ensure equitable health outcomes for all.
Our center is designed to begin as a hub for maternal and infant health equity. We will offer services such as virtual monitoring, RN-Doula support, educational resources, and healthy food options. By using advanced technology and creating a supportive community environment, our goal is to have a significant impact on maternal and infant health outcomes in St. Louis and rural Missouri. We envision St. Louis as the focal point for solving this national disparities healthcare crisis.
Why I Am Doing This in Honor
of My Grandfather:
My grandfather, Whitey Herzog, was not only a legendary MLB manager for the St. Louis Cardinals but also a person of immense integrity and compassion. His dedication to the community and his unwavering commitment to helping others left a profound impression on me. Creating the Herzog Center for Connected Care is my way of honoring his memory and continuing his legacy of giving back.
Whitey believed in the power of teamwork, perseverance, and making a difference in the lives of others. These values have guided me in my journey and have been the cornerstone of my efforts to improve healthcare access and equity. By naming the nonprofit after him, I hope to inspire others with his legacy and remind them of the positive impact one can have through dedication and compassion.
Thank you once more for your interest. I am genuinely enthusiastic about the promising future of the Herzog Center for Connected Care and the meaningful, positive changes we can bring to our community.
